How to select District and Scheme :
** Select the district from ‘District Name’ drop down
** Select the scheme from ‘Scheme Name’ drop down
** Clicking on ‘Go To Fill Your Application’ button will redirect you to the relevant application form what you select under ‘Scheme Name’

How to fill the Revolving Fund to SHGs Form :
** Enter the name of Self Help Group
** Enter the number of members in the SHG
** Enter the certification of Grading-Grade/Marks
** Mention the Date of issue of Grade by clicking on date picker
** Enter the bank account number
** Mention the date of opening of bank account by clicking on date picker
** Mention the date of grading by clicking on date picker
** Enter the group has been recommended for financial assistance or not
** Select the bank name from the ‘Name of the bank’ drop down
** Select the branch name from the ‘Branch Name’ drop down
** Mention the activity for which financial assistance is required
** This amount will be displayed automatically
** This amount will be displayed automatically
** This amount will be displayed automatically
** This amount will be displayed automatically
** Enter the certification on grading of the group
** Enter the certification on grading of the group
** Mention the Project report date by clicking on date picker
** Enter the details of copy of bank account/pass book statement
** Enter the name of a member of SHG
** Enter the member’s either father or husband name
** Mention the date of birth of a member
** Enter the Family Card No with issued date of a member
** Enter the community certificate with issued date of a member
** Enter the annual income certificate no with issued date of a member
** Enter the voter ID with issued date of a member
** Upload the member photo by clicking on ‘Browse’ button
** By Clicking on ‘Add Member’, you can add another member. By clicking on ‘Remove member’, you can remove a added member but minimum one member details need to be presented.
** Read declaration and enter the same letters you see in green box.
** After these, you have to verify all the details you entered in above form and press submit button.
** Finally, you will be redirected to acknowledgement page with your acknowledgement no.
