Job Description:
Health Care Advisors will provide telephone-based health advice, education and support services for vulnerable patients suffering from chronic diseases, living in different parts of the world. These care advisors will help patients navigate complicated care delivery systems and motivate healthy behaviors to improve health outcomes and avoid medical catastrophes.

Work in a telephonic care center to assess to provide health advice and education to patients from across the globe, particularly in India and United States.

Collaboratively work in a team to research relevant clinical guidelines to maintain a comprehensive health care database.

Track health status and clinical indicators through follow-up calls and through the use of tele-medicine technologies for direct bio-metric monitoring, as appropriate

Engage patients, family members, caregivers and physicians in meaningful phone interactions to assure a well-coordinated care plan is established and implemented for each person enrolled in the program

Regularly assess the effectiveness and quality of services provided to patient by analyzing data reports and implementing continuous improvement strategies.

Desired Candidate Profile:
At least 1 year of experience in interacting and communicating with individuals with gaps in knowledge (healthcare or otherwise).

Exceptional English speaking skills with ability to engage in two way conversations with individuals from around the world. Ability to speak Spanish is a plus.

Computer skills are a plus for quick and effortless documentation and monitoring.

A sensitive, caring personality and strong commitment to compassionate and holistic care to individuals from all backgrounds is a must.
