Application for B. PHARM (Lateral Entry) (D. PHARM TO B. PHARM ) 2017-2018 session

Applications can only be downloaded from the websites .tnhealth.org, 2) tnmedicalselection.org and the filled in application along with necessary enclosures should be sent in a large sized cover measuring 38cm X 28cm superscribed "APPLICATION FOR B.PHARM (LATERAL ENTRY ) COURSE 2017 – 2018 SESSION"to the Secretary, Selection Committee, Kilpauk, Chennai -10 before the last date along with a Demand Draft for 350 /- which is non-refundable towards the cost of application form drawn in favour of “The Secretary, Selection Committee, Kilpauk, Chennai-10” and payable at Chennai.
The name of the candidate and the name of the course should be written on the reverse of the Demand Draft. The template with requisite details available for download should be printed and pasted on the cover.
SC/ SCA and ST candidates who are natives of Tamil Nadu are exempted from payment of the cost of the Application Form.
GENERAL INSTRUCTIONS:-
a) Candidates seeking admission to B.Pharm (Lateral entry) should submit the application form with necessary enclosures which should reach:
The Secretary,
Selection Committee, No.162,
Periyar E.V.R High Road, Kilpauk,
Chennai – 10.
on or before 22.09.2017 before 5.00 p.m.
b) The candidates shall ensure that the completed application form with all particulars and enclosures reaches the Selection Committee within the date and time specified. Applications received by Speed post / courier or any other means, after the last date & time will not be accepted irrespective of the date of booking.
c) The Selection Committee shall not be responsible for any delay in the receipt or for the loss in transit of application form etc.,
d) Request for extension of time for submission of documents called for, after the specified date and time will not be entertained.
e) Any supporting documents received after the due date or application without required documents / particulars will not be entertained.
f) No oral enquiry would be entertained.
g) Photocopy/ Fax copy of application will not be accepted.
h) Candidates seeking admission under Special Category under Orthopaedically Physically Disabled shall submit a separate Special Category Form along with the main application in the prescribed format available along with an additional fee of Rs.100/- by way of a Demand Draft enclosing with the relevant certificates.
If the Certificates are not enclosed by the candidates, they will not be considered under the special category without any intimation to the candidate.
i) Candidates selected under General category will not be allowed to claim very same status as the candidates selected under special category and vice versa.
j) Both upper limbs, vision and hearing should be normal for all candidates including for candidates applying under orthopaedically physically disabled category.
k) All the Demand Drafts should be drawn in favour of the Secretary Selection Committee, Kilpauk, Chennai-10. (Payable at Chennai)
EDUCATIONAL QUALIFICATION:
a. Candidates should have passed in H.S.C. qualifying Examination conducted by the Tamil Nadu State Board or any other equivalent examination.
b. Candidates should have passed Diploma in Pharmacy course.
MINIMUM ELIGIBILITY MARKS :
i. (a) For OC/BC/BCM/MBC/DNC CATEGORIES:
Candidates should have aggregate of 40% marks in the first and second year D.Pharm. Examination with a pass in H.S.C.
(or)
Individual pass mark in Physics, Chemistry, Biology or Botany & Zoology or Mathematics both in theory and practical with not less than 40% aggregate marks in the above subjects in the qualifying H.S.C. examination (Academic stream) with a pass in D.Pharm Course.
(b) For SC/SCA/ST CATEGORIES:
Candidates must have obtained individual pass marks in Physics, Chemistry, Biology or Botany & Zoology (or) Mathematics both in Theory & Practical with a minimum of 35% marks in the above subjects with a pass in D. Pharm Course.
ii) Candidates who have passed Diploma in Pharmacy should have completed 500/750 hours of practical training on or before 31.5.2017 and should have obtained the provisional certificate for having passed the Diploma Examination.
iii. D.Pharm. qualified candidates who are serving in Tamil Nadu Medical Services will be treated as Service candidates for selection under the seats reserved exclusively for service candidates.
a) Service candidates serving in Tamilnadu Medical Service on a regular basis should have completed two years of service as on 31.5.2017 alone excluding the leave for which the candidate is eligible/availed other than casual and compensatory leave will be considered.
b) D.Pharm. qualified candidates who are approved probationers serving in local bodies will be considered as service candidates and must have completed 2 years of service as on 31.5.2017.
c) Service candidates should send their application through the Head of the Medical College Hospital/ Institution in which they are presently working.
d) Service candidates taking allotment in Self-financing colleges will not be paid salary and they have to avail leave to which they are eligible for the period of study on their own after obtaining prior permission from their Head of the Department concerned.
e) Candidates who are serving purely under “temporary category” in Tamilnadu Medical Service will be considered for selection under open quota as non-service candidates only.
PROCEDURE FOR FILLING AND SUBMISSION OF APPLICATION:
i) a. Before filling up the application form and scrutiny form, read all the instructions carefully.
b. Fill up the Application form and scrutiny form legibly, neatly and correctly. The information will be processed by using a computer. Any mistake committed by the candidate will prejudice the selection.
c. Cutting, overwriting and erasing in the Application form and in the scrutiny form should be strictly avoided.
d. Application with incomplete or insufficient particulars or without enclosures or those received after the last date mentioned will be summarily rejected.
e. Filled up application without the signature of the candidates will be rejected.
(ii) All the entries in the application form should be written legibly by the candidate in his / her own handwriting. The candidate should ensure that all information including the marks in the qualifying examination furnished by him/her in the application and the enclosures are correct.
The candidate is informed that if after scrutiny of his / her application it is found that the marks furnished by him / her in the application are not correct or any other information is found to be incorrect, then;
a. He/she will forfeit the admission no matter at what stage of the course he/she will be at that time.
b. He/she will be debarred from pursuing any studies for a period of three years.
c. Legal action will be initiated against him / her for furnishing incorrect information.
d. Criminal action will be initiated against him / her for furnishing bogus mark sheets / bogus Community certificate/false nativity certificate.
e. Do not make hole, use pin or stapler on the scrutiny form. Do not fold any of the forms under any circumstances.
