Name Change Certificate :
Any person residing in the State of Telangana who intends to change his name shall apply, in the first instance, in the form in Annexure-I to this Order, to the District Magistrate, the Additional District Magistrate or an Executive Magistrate having jurisdiction, for issue of an official communication in the form of a letter or memorandum to the effect that he is a Citizen of India.

Where To Apply :
The citizen can apply for the above at the nearest eSEVA centres identified for their respective mandals.

The citizen must apply for Name change with the following enclosures / documents :
** Two passport size Color photos
** Affidavit (In case of MINOR father has to sign the application & affidavit)
** Police station certificate & Character certificate
** Citizenship certificate
** School Bonafied certificate (Attested by Gauzetted officer)
** Transfer Certificate
** Ration card copy
** Service Certificate
** No objection certificate from head of the Department

Service Charges :
The citizen has to pay Rs. 200 / - at the eSEVA Centre at the time of applying for the Name Change.

Application Status :
The status can be verified by the citizen by using the ‘Know your status’ option in the website of Hyderabad Collectorate by entering Collectorate Inward No and Year given by eSEVA.

Where to collect the Certificate :
The citizen has to collect the certificate from the same eSEVA center where it is applied as per the date given in the acknowledgement.
