Old Age/Widow/Disability Pensions :
This scheme is introduced to give financial assistance to the needy old/Widow/Disabled.
There are certain criteria which should be fulfilled in order to get the benefits of the Scheme.

List of documents to be attached with the form :
The prescribed application forms are either available online or he/she can approach to District Office centre directly. Also, you will have to provide following supporting documents
(1) Income Certificate
(2) Photocopy of Degree of Disability Certificate by Medical Board (In case of Disability Pension)/Handicap Certificate
(3) Photocopy of Identity Proof : EPIC/Pan Card/Photocopy of Identity Proof : EPIC/Tax Paid Receipt/Driving License/Defense ID Card/Govt. ID Card/Ration Card
(4) Residence proof/Voter Id
(5) Passport/Photocopy of Certificate of age (In case of Old Age Pension) : EPIC/Age Certificate
(6) Husband Death Certificate/Photocopy of Death Certificate of the Husband by Gram Pradhan/Municipality councillor (In case of Widow Pension)
