UG Admission - 2014 (B.Sc.Nursing, BPT and BPO) :
Rajiv Gandhi University of Health Sciences, Bangalore is inviting applications from the eligible candidates who fulfill the minimum qualification and other conditions for admission to B.Sc. Nursing, BPT & BPO (Bachelor of Prosthetics & Orthotics) courses in Government Colleges and for Government seats in Private Colleges in Karnataka for the academic year 2014-15.

ELIGIBILITY :
1) Applicant should be a citizen of India.

2) A candidate should have studied and passed in one or more Government or Government recognized education institutions located in the State of Karnataka for a minimum period of SEVEN academic years commencing from I Std. To II PUC or 12th Standard as on 1st July of the year and should have appeared and passed SSLC or 10th standard or II PUC or 12th standard or equivalent examination from institution located in the State of Karnataka.

Provided that candidate who takes more than one year to pass a class or standard, the years of academic study is counted as one year only.

3) A candidate should have studied and passed I and II year Pre-University Examination within the State of Karnataka from an educational institution run or recognized by the State Government and that either of his parents should have studied in a education institution run or recognized by the State Government and located in the State of Karnataka for a minimum period of SEVEN years.

4) Have been born on or before 31/12/1997.

Certificates to be enclosed along with the Application Form:
(A) The candidate has to submit attested photo copies of the following documents duly attested by Gazattee Officer along with the filled application form.
a. S.S.L.C marks card (for proof of Date of Birth).
b. Marks Card of II-PUC or equivalent examination.
c. Prescribed certificates in the case of candidates claiming reservation under different categories i.e., category & income certificate issued by the Tahasildar of Karnataka only.
d. Domicile Certificate for 10 years in Karnataka (Issued by the Tahasildar of Karnataka) or Study Certificate of seven years in Karnataka.
e. Relevant Certificates for those who are claiming exemption for seven years study in Karnataka.
f. Prescribed fee in form of Demand Draft.
g. Candidate studied other than Karnataka PUC Board, CBSE Board and ICSC Board should submit the Eligibility Certificate obtained from the Rajiv Gandhi University of Health Sciences, Bangalore without fail.

(B) The following original documents to be produced at the time of allotment:
1. All the above certificates in original.
2. Attested Photo Copy Transfer Certificate.
3. Physical Fitness Certificate.
4. Category Certificate issued by the Tahasildar for candidates claiming reservation under different categories in the prescribed form.
5. Study certificate for having studied in Rural/Kannada medium.

APPLICATION FORM :
a. The application form can be downloaded from the Rajiv Gandhi University of Health Sciences website www.rguhs.ac.in from 16/07/2014.

b. How to submit:
i. In Person: The application filled in all respects along with the attested copies of all the required documents may be submitted for registration, in person, at Rajiv Gandhi University of Health Sciences, 4th ‘T’ Block, Jayanagar, Bangalore – 560 041.

ii. By Post: The application filled in all respects along with the attested copies of all the required documents may be sent by post for registration to Registrar, Rajiv Gandhi University of Health Sciences, 4th ‘T’ Block, Jayanagar, Bangalore – 560 041. Candidates should superscribe on the cover as “APPLICATION FOR UG COURSES – 2014”.

REGISTRATION FEE :
(a) The candidates are required to send the Demand Draft of Rs. 400/- (Rupees Four Hundred Only) for General Candidates and Rs. 100/- (Rupees One Hundred Only) in case of SC/ST and Category – I through demand draft drawn in favour of Registrar, Rajiv Gandhi University of Health Sciences, Bangalore.

(b) Separate application forms and separate Demand Drafts should be submitted in respect of the candidates who are applying for all the courses i.e. B.Sc (N) & BPT courses.

2. Launching of application 16/07/2014
3. Last date for receipt of applications at RGUHS 30/08/2014, 5-00 P.M.
4. Announcement of provisional merit list 05/09/2014
5. Last date for submission of application for any corrections in the provisional merit list 08/09/2014
6. Announcement of final merit list 16/09/2014
7. Tentative dates of counseling Will be announced later in the website
8. Commencement of Academic Year 17/10/2014
