Birth certificate/Birth correction :
Information required for obtaining the birth and death certificate are:
Details :
** Enter Child Name
** Enter Date of Birth
** Enter Sex
** Enter Mothers Name
** Enter Birth of Place
** Enter Address
** Filling up the specified forms.

Death certificate/Death correction :
** To collect the Death Certificate in case of funeral in soil or burn in the village certificate must be brought from the Chief of the Village. If it is the Municipality area then Certificate must be brought from the Municipality with the names of parents
** If the area is under Kayalpatnam Municipality then the Certificate from the Municipality must be collected.
** Original Death Certificate must be brought with.
** If the Chief does not mention the name of mother in that case the voter card or the ration card of mother must be brought.

The Kayalpatnam Municipality open the session in 1952 with the title of grade Town Panchayat. From 1982 with the title of Selection grade Town Panchayat. After 14.06.2004 it upgraded to III Grade Municipality G.O. Ms. N0. 270(MA & WS) Dt : 11.06.04.. The area of the Municipality is 12.50 Sq.Km.
