Application are invited from local candidates for filling up the following posts mentioned below on consolidated pay @ Rs. 7000/-p.m. in terms of Notification No : 489/GEN/DOP dated: 31.10.2012 under the Health Care, Human services and family welfare department, till the time he/she crosses the probation period.

1.Name of the post : Multi Purpose Health worker (made) Grade-lll

Educational and technical qualification : Class X passed from recognized Board with MPHW (male) training course from recognized institute

2.Name of the post : Multi Purpose Health worker (female) Grade-lll
No. of post : 18
Educational and technical qualification : Class X passed from recognized Board with Multi Purpose Health worker (Female) training from recognized Nursing Council

3.Name of the post : Medical Record Technical Grade-lll
No. of post : 01
Educational and technical qualification : Class X passed from recognized Board with Certificate course in Medical Records Technician.

4.Name of the post : Dental Assistant Grade-lll
No. of post : 01
Educational and technical qualification : Class X passed from recognized Board with Diploma in Dental Assistant or should have undergone MPHW (male/Female) training conducted by the department or any Nursing Council

Age Limit :- 18-30 years as on 31.10.2012

Selection :
Selection will be made through open Competitive Examination. Syllabus for the written examination for each posts can be obtained from the Office of the undersigned at the time of collection of Application form

Fee :
Candidate seeking admission for the examination should obtain the prescribed form from the office of the Joint Secretary-ii, Health care, Human services and family Welfare Department, Health Secretariat, Gangtok on the production of Bank Receipt for Rupees 100/- (Rupees One Hundred) only credited under head 0210-OR (cost of application form).

How to Apply :
i) Application forms shall be available at any working day from 10 :00am to 3:00p.m. From 17th September, 2012 to 27th September, 2012.
ii) Application form complete in all respect along with attested photocopies of the following document, must reach to the office of the

Note :
Documents Needed :
1. Class X Passed Mark sheet and Certificate onwards;
2. Valid Local Employment Card in case of unemployed candidate;
3. SSC/Certificate of Identification as the case may be; SC?ST and MBC?OBC Certificate as the case may be;
4. No objection certificate from the Secretary/Heads of Department in case of in-service candidate.
5. Husband’s Sikkim Subject Certificate/Certificate of Identification in case of married woman.
6. Unmarried Certificate issued by SDM/BDO in case of unmarried woman.
