The prime responsibilities will include :-

* To register the new beneficiaries
* Lead person in the home care team
* Visit patients at home to offer nursing, psychosocial and emotional support as per the set protocol
* Maintain records of home visits
* Maintain the various registers used in the home care program
* Refer patients to the physician/seek advice as and when required
* Liaison with the social worker and trained volunteers

The successful candidate should be :-

* GNM with 3 to 4 years of experience.
* Should have excellent communication skills
* Should be sensitive to the cause of the organisation
* Should have an eye for detail
* Should have sense of responsibility
* Should be good at record keeping and generating reports
