WBPGMAT / WBPGDAT - 2015 :
WBPGMAT :
Admission to PG seats in the Command Hospital, Kolkata-700027, is open to candidates selected by DGAFMS through Combined Admission Test conducted by DGAFMS. Admission to a certain number of PG seats in the Command Hospital, Kolkata as specified by DGAFMS, shall be made through WBPGMAT-2015. [Vide DME (WB) letter no. DME/ SPL. Correspondence/354, dt. 17.1.2007]

i. Citizen of India and permanent resident/domicile in West Bengal who have passed Secondary or its equivalent and Higher secondary or its equivalent from institutions in West Bengal and have obtained the M.B.B.S degree from any University of the State of West Bengal and they have completed or schedule to complete their rotating internship for one year before 31 March 2015. He/she should possess provisional/permanent registration number from WBMC.

ii. Citizen of India who have passed secondary or equivalent and higher secondary or equivalent both from the institutions in the state of West Bengal but obtained MBBS Degree from any University other than that of West Bengal that is from any other state /UT from India or abroad and recognized by Medical Council of India and have completed one year compulsory rotating internship in MCI recognized institute of India and have Medical Council of India’s permanent registration number and candidate/parent is permanent resident/domicile in West Bengal

iii. Citizen of India who have not passed secondary or equivalent or higher secondary or equivalent from the institutions in the state of West Bengal and have obtained MBBS degree from any University in India or abroad recognized by Medical Council of India and have completed one year compulsory rotating internship in MCI recognized institute of India and have Medical Council of India’s/respective state medical council permanent registration number and candidate/parent is permanent resident/domicile in West Bengal.

iv. The candidates with MBBS degree and in permanent Government service in West Bengal other than WBHS/WBMES/WBPH&AS services under Health and Family Welfare Department irrespective of state of domicile or residence will be eligible to apply provided the present employer allows him/her to pursue the course during total training period. Such candidate if resigns from the service anytime during the process of examination to selection and pursuing the course, his/her candidature will be cancelled outright.

WBPGDAT :
Eligibility criteria :
i. Citizen of India and permanent resident/domicile in West Bengal who have passed Secondary or its equivalent and Higher secondary or its equivalent from institutions in West Bengal and have obtained the B.D.S degree from any University of the State of West Bengal and they have completed or schedule to complete their rotating internship for one year before 31 March 2015. He/she should possess permanent registration number from WBDC/DCI/other council except candidates who are pursuing internship after passing BDS from the colleges affiliated to WBUHS .

ii. Citizen of India who have passed secondary or equivalent and higher secondary or equivalent both from the institutions in the state of West Bengal but obtained BDS Degree from any University other than that of West Bengal that is from any other State /UT from India or abroad and recognized by Dental Council of India and have completed one year compulsory rotating internship in DCI recognized institute of India and have Dental Council of India’s permanent registration number and candidate/parent is permanent resident/domicile in West Bengal

iii. Citizen of India who have not passed secondary or equivalent or higher secondary or equivalent from the institutions in the state of West Bengal and have obtained BDS degree from any University in India or abroad recognized by Dental Council of India and have completed one year compulsory rotating internship in DCI recognized institute of India and have Dental Council of India’s/respective state Dental council permanent registration number and candidate/parent is permanent resident/domicile in West Bengal.

iv. The candidates with BDS degree and in permanent Government service in West Bengal other than WBDS/WBDES services under Health and Family Welfare Department irrespective of State of domicile or residence will be eligible to apply provided the present employer allows him/her to pursue the course during total training period. Such candidate if resigns from the ervice anytime during the process of examination to selection and pursuing the course, his/her candidature will be cancelled outright.

Open Category candidates, if selected have to sign a bond for rendering service for t hr e e ye a r s after completion of course and training in PG degree as notified by Government of West Bengal vide its memo no. HF/O/MERT/923/ME/MISC-78-13 dated 10.06.2014 Copy is annexed with this booklet.

Application for admission to MDS Courses will not be accepted unless the candidates have got themselves registered with permanent registration number given by the West Bengal Dental Council or Dental Council of India or any other council. But the candidates, who passed BDS from affiliated colleges of WBUHS and their internship is to be completed on or before 31 March, 2015 will not require putting registration number.

Steps for filling up On-line Application Form :
First step – Registration – Candidates will fill up basic details in the form. Identification number will be generated. User Name and pass word will also be generated and will be communicated to his/her email.

Second step – Log in with user name and pass word and then change his or her pass word. User name and pass word must be kept confidential and ready for use.

Third step – Fill On-line application form. Be careful in selecting/writing option/s and put the appropriate data. The documents relating to permanent residency /domiciled status / caste attachment documents/ other relevant documents are submitted. If he or she is found eligible after publication of result. Mere appearing at the examination will not give right for selection if the requirements in the Information Booklet is not matched/ tallied. Inputs submitted if found incorrect through omission or commission will be viewed seriously and the process of selection will be rejected at any time even after admission.

Fourth Step – (a) Using Identification number, if a candidate opts for the online payment then instant generation of payment receipt (via bill desk) will be available after successful transaction.
(b) Off line generation e-challan is also available for the candidate as an alternative option. After payment in any branch of United Bank of India with the print copy of the e challan the candidate has to view payment status and get Transaction ID. Payment made by 4.00 pm will be updated on the same day after 8.00 pm. For off-line and online payment Rs. 20/- will be paid for bank charges.

Fifth step - After confirmation of payment (online instant and offline after 8 pm on the day of payment) candidate will be able to upload his/her photo and signature.

Sixth step – Take print out of the form and make it photocopied. Keep one photocopy for himself or herself and in the other copy, paste similar photo of appropriate size as mentioned earlier, sign it and then submit to the University by hand or speed post within time frame as stated in the Information Booklet.

Submission of print-copy of forms at W.B. University of Health Sciences (except Saturdays, Sundays and Holidays) 17.12.2014 to 05.01.2015 (11.30 a.m. to 4.00 p.m.)
Print-copies will not be received in the office of the WBUHS in person after 05.01.2015. ( 4 P M) However, print-copies sent by Speed/ Registered post will be received up to 07.01.2015. The print copies must contain proof of online/offline payment.
Admit card to be downloaded 27.01.2015 ( 6 P M ) onwards
Date of Admission Test WBPGMAT -2015 On Sunday 01.02.2015 (12 noon to 2.40 p.m.)
Declaration of result WBPGMAT–2015 On or before 12.02.2015
Date of Counselling To be notified in wbuhsexams.in on or before 01.03.2015
