APPLICATION :
1. Prescribed application forms can be obtained from the Notified Centers against DD drawn on the Nationalized Bank in the name of the "AMUPMDC" and payable at "Mumbai" for Rs. 1000/- (Rupees One Thousand Only) Refer Annexure “B”.

2. Application forms will also be available from Association’s Office, AMUPMDC, Shrijee House 75 Mint Road, Mumbai – 01 by post with an additional DD of Rs. 100/- for postal charges.

3. Forms will also be available from Selected Branches of Punjab National Bank (PNB) as mentioned in Annexure “B” on cash payment of Rs. 1000.00.

4. The Application Form must be filled by the candidate in his/her own handwriting using only Black Ballpoint Pen. The application form complete in all respect shall be submitted directly to the office of AMUPMDC, Shrijee House, 75 Mint Road, Mumbai 01, either in person or by Speed Post only No Private Courier will be accepted. The filled-in Application Form is to be duly returned in the Envelope provided by AMUPMDC with the following documents

1.1. Filled-in Application Form
1.2. DD for Rs. 2000/- (Rs Two thousand only) drawn in favour of "AMUPMDC" and payable at Mumbai (for Asso-CET 2013 Examination Fee).
1.3. Receipt-Cum-Identity Card, Annexure - “F” (upper portion)
1.4. Self Address Envelope with appropriate postal stamp (for sending back the receipt)
1.5. Application From should be submitted within the time in advance. AMUPMDC will not be responsible for postal delay.
1.6. Application Form Submitted by Speed POST must be accompanied by a selfaddressed and stamped envelope along with Receipt-cum-Identity Card (Annexure - “F” ) for acknowledgement.
1.7. Incomplete application forms will be rejected.
1.8. The official at the AMUPMDC office while accepting the application form submitted in person shall issue the Receipt cum Identity Card attesting the photograph of the candidate with signature and stamp.

Qualification :
For MBBS, BDS and BAMS :
A candidate belonging to open category must have obtained not less than 50% Marks in Physics, Chemistry & Biology subject taken together at the HSC (or Equivalent) Examination.

A Candidates belonging to Reserved Category must have obtained not less than 40% Marks Physics, Chemistry & Biology subject taken together at the HSC (or Equivalent) Examination.

For B. Sc (Nursing):-
A candidate belonging to open category must have obtained not less than 45% Marks in Physics, Chemistry, Biology & English subject taken together at the HSC (or Equivalent) Examination.

A Candidates belonging to Reserved Category must have obtained not less than 40% Marks Physics, Chemistry, Biology & English subject taken together at the HSC (or Equivalent) Examination.

For BPTh, BOTh, BASLP rule no. 5.5.1, 5.5.2 and 5.5.3 is not applicable to this course, passing at Std. XII or HSC or equivalent examination is necessary.
