iii) PLEASE ENTER THE CHARACTERS AS SHOWN IN THE IMAGE BELOW
iv) Click on the Submit Button

3) If you Select BY NAME
i) ENTER YOUR NAME
ii) ENTER YOUR FATHER'S NAME
iii) SELECT DATE OF BIRTH ( DD/MM/YYYY )
iv) PLEASE ENTER THE CHARACTERS AS SHOWN IN THE IMAGE BELOW
v) Click on the Submit Button
