[B][U]GENERAL INFORMATION[/U][/B]:
1. The number of seats/posts advertised may increase or decrease.
2. Reservation will apply as per rules and admissible only to candidates domiciled in the state of Uttar Pradesh. Candidates domiciled elsewhere will be treated as General category.
3. Candidates must submit copy of Medical Registration from Medical Council of India or State Medical Council / proof of recognition of his/her degree from Medical Council of India or with State Medical Council, failing which application will be rejected.

4. The last date for payment to fill ONLINE application form is 20th November 2017, 17:00h IST & last date for submitting on line application forms is 22nd November, 2017, 17:00h IST The candidates must ensure that they fill correct and complete information.

Based on the information filled by candidates ONLINE, their suitability for appearing in the entrance examination will be assessed. The Institute will not be responsible if application of any candidate is rejected on the basis of information filled ONLINE.

5. While filling ONLINE application forms, candidates must follow the instructions on the screen.
6. Candidates must use their correct transaction number and date of birth while filling online application form. No telephonic / personal request in this regard will be entertained.
7. Incomplete application and application received after the last date will summarily be rejected.
8. In case of any dispute, the decision of the Director, SGPGI, Lucknow will be final.

9. The completed application along with enclosures (print out of application form (all 3 pages) filled ONLINE along with all necessary documents) should be received latest by 25th November, 2017 through courier or speed post in a envelope superscribed “Application for entrance exam of SR(HS) / MPR / Demonstrator (… name of specialty …) along with advertisement number and addressed to Executive Registrar, SGPGI, Raebareli Road, Lucknow 226014.

[B][U]Filling the application form [/U][/B]:
** Open the home page of SGPGI at sgpgi.ac.in 
** Click on the link that announces the admission to SR(HS) / MPR / Sr Demonstrator / PDC and a separate one for PDF courses
** On the “Online form submission” tab, click on “Register and Pay Online”. The State Bank of India, ‘State Bank Collect’, page will open. Accept ‘Terms and Conditions’, and click on ‘Proceed’.
** Select “Category” as application fee, select the correct “Advertisement Number”, fill details of payment and submit. At this stage, you will be required to provide your name, date of birth, 10-digit mobile phone number, email ID, father’s and mother’s names. Be sure to fill these particulars carefully / accurately and give a mobile number and email ID that you regularly use.
** While filling your online registration slip / application form. It is your responsibility to remember your registered email-id, mobile number and bank transaction number beginning with ‘DU’ and date of birth. No telephonic /personal request, in this regard, will be entertained.
** Take print out of your SBI collect transaction number. This transaction number will be used for filling online application form, eligibility status, download admit card etc.
** Verify details and confirm transaction
** Pay fees using options provided (Net banking, credit card, debit card or challan paid in a branch of State Bank of India).
** A transaction alert will be sent to your registered mobile number
** After one clear working day (for instance if you made payment on working day 1, then should log in on working day 3; Sundays and holidays do not count), log onto SGPGI website at sgpgi.ac.in 2. On the tab, “Online form submission”, select “Complete form”
** Fill the transaction number and your date of birth
** The main form will open, partially populated with some personal details. Complete it. Click on “Submit once and please wait”, and upload documents. Online form submission is mandatory for claiming eligibility. Submission of only hard copy will not be considered sufficient for claiming eligibility.
** You will require to have scanned copies of your recent photograph, signature and all relevant documents as separate files which you need to upload (PDF/JPG formats).
** When entry of data and uploading of documents has been completed, click on “Back to print documents”. Then, print pages 1, 2 and 3, and send these along with self-attested supporting documents posted to the Executive Registrar as explained in the next section.

Then accept ‘Terms and conditions’ and click on ‘Proceed’. For ‘State’, choose ‘Uttar Pradesh’, and for ‘Type of corporate/institution’, choose ‘Educational institutions’, and for Educational Institutions Name, choose ‘SGPGI’.

Then choose Payment category as ‘Application fee’, select the correct ‘Advertisement number’, fill details and proceed to the subsequent steps.
