[B][U]Health Card Modification Procedure [/U][/B]:
1. Login as Employee. Navigation : Registrations → Download Health Card. It displays 'Download Health Card' option in Employee login.
2. On clicking 'Download Health Card' tab, a page will be displayed wherein the user has a provision to 'Edit Card Details' as shown.
 
3. On clicking 'edit card details', a page will be displayed . Click on 'Validate'.
4. Here the user(employee) has a provision to edit the address details, photo and gender type(except name)

5. On clicking 'submit' button, a message 'Details submitted successfully for verification' will pop up.
NOTE : Card details get updated only after the verification by Trust.

6. Once the card details are verified by trust, the same will be reflected for beneficiaries as well.
NOTE :  Beneficiaries have a provision to edit the photo & gender type.

[B][U]Employees Health Scheme [/U][/B]:
Employees Health Scheme is formulated to provide cashless treatment to the employees, pensioners of the State Government and their dependent family members, which will ipso facto replace the existing medical reimbursement system, with additional benefits such as post-operative care and treatment of Chronic diseases, which do not require hospitalization and treatment in empanelled Hospitals.

[B][U]Important Stakeholders in the Scheme [/U][/B]:
[B]Beneficiaries [/B]:
The Scheme is intended to benefit Serving employees and Retired employees of the state.
[B]Aarogyasri Health Care Trust [/B]:
Aarogyasri Health Care Trust will implement the Scheme under the supervision of State Government.

[B]Network Hospitals - Service Providers [/B]:
A health care provider shall be a hospital or nursing home in Telangana established for indoor medical care and treatment of disease and injuries and should be registered under Telangana Private Allopathic Medical Establishments (Registration & Regulation) Act and Pre-Conception and Pre-Natal Diagnostic Techniques Act (Wherever Applicable).

[B][U]Enrolment [/U][/B]:
  [B][I]  How to enroll in EHS as an employee?[/I][/B]
        No separate enrolment process is needed for an employee. Based on the employees data (including Aadhar Data) furnished by the DDOs to the Finance Department under CFMS, which is in turn is made available to the Aarogyasri Health Care Trust by the Finance Department to generate cards and position in the login of the employee in EHS portal, which can he downloaded, printed and used by the employees and their dependent family members.

[B][I]  How to enroll as a pensioner in EHS?[/I][/B]
        a) Login to the web portal ehf.telangana.gov.in.
        b) Your username = p + STO ID + PPO ID. Ex: If your STO ID is 230 and your PPO ID is 012, then your username = p230012. You can obtain your username and password from your STO/APPO or by calling 104-sevakendram by dialing ‘104'.
        c) Sign-in with your username and password. Submit the application.
        Applicant will receive health card after approval by the Trust. The card enables the beneficiary to avail the scheme benefits from the list of Empanelled Hospitals, for the specialties empanelled under EHS.

[I][B] Is Aadhaar card or Enrollment slip compulsory for enrolment under EHS?[/B][/I]
        Yes. Even if Aadhaar Card enrollment number is available, it is sufficient to enroll under EHS. Please read the instruction on how to fill Aadhaar enrolment number in the application form, Consisting 28 digit number.

    If a pensioner has not enrolled himself/herself under the scheme, does he/she have to contribute for the scheme?[/B][/I]
        Yes. The deduction of contribution to the scheme will be done automatically by STO/APPO from the November pension payable on 01.12.2014 and onwards, as per G.O.Ms. No.134, HM & FW (I.1) Dept., DT: 29.10.2014, irrespective of whether he/she has enrolled in the scheme or not.

[I][B] What is the employees/pensioners subscription under the Scheme:[/B][/I]
        No contribution to be made by the employees/pensioners.
    A State Government Employee/Pensioner has enrolled under the Scheme and added his/her spouse, who is also a State Government Employee/Pensioner, as dependent family member. In that case whether contribution will be deducted from the salary/pension of the other spouse, who is added as dependent family member.

As per Para G.O.Ms. No.32, HM & FW (A2) Dept., DT: 03.11.2014, the total cost on implementation Employee Health Scheme shall be borne by the Government. No contribution will be collected from the Employee/ pensioner.

[I][B] Is a single card like ration card be given for the entire family under the scheme?
   [/B][/I]     No. Individual cards will be issued to the employee/pensioner and their dependent family members. No contribution required from the employees/pensioners

[I][B]  From where the Health Card will be issued to the Pensioner?[/B][/I]
        Health Card will be generated by the Trust and placed in the Login of the Pensioner. The Pensioner can login to the portal by using his/her "User ID" and "Password", download the Health Card, Print it and use it for availing treatment in the Network Hospitals under Employees Health Scheme.

[I][B]Is unemployed daughter, if not married, eligible under the scheme?[/B][/I]
        Yes. Unemployed daughters who are unmarried or widowed or divorced or deserted are eligible. Once she is married she becomes ineligible.

[I][B]    Who is competent to delete name of "son" who has crossed the age of 25 years?[/B][/I]
        The date of birth of "son" furnished by the employee/pensioner is available in the System. As such, the system automatically renders the "Son" who crosses 25 years, ineligible under the Scheme and invalidates his Health Card.

[I][B] Can the Employee edit the employee details on his health card?[/B][/I]
        No. The employee cannot edit the details like name of the employee; date of birth, designation, department on his health card, as the details of all the employees are received from Finance department (CFMS) and the same is authenticated by DDO Concerned.

However, Change of fields like name, designation, department etc., if any to be changed, the employee shall submit details with approval of HOD through email to ehfts[[AT]aarogyasri.gov.in

[B][I]Can the employee add details of dependents, which are not included in CFMS data?[/I][/B]
        No. The employee can get them included in CFMS data or furnish details through Email ehfts[AT]aarogyasri.gov.in with an authentication from HOD concerned.

[I][B]    I forgot my password. How to reset new Password?[/B][/I]
        After clicking on Sign-in button on home page, click on 'forgot password'. The system generated password will be sent to the applicant's mobile number and e-mail id.[/QUOTE]
