    A current resume/CV (2 pages max.)
    A statement outlining your educational beliefs/philosophy (1 page max.)
    The names, current email addresses, telephone numbers, and positions of three confidential referees (two of whom should have served as your supervisor and one as a professional colleague)
    Up to three scanned letters of recommendation (optional)
