1. Death certificate of the deceased person (1st August 1998 onwards)
2. Certificate from the concerned PHC/Ward Commissioner certifying that the deceased was a person below the poverty line and main family earner. 
3. Income Certificate of the applicant 
4. Age proof certificate of the applicant 
5. Other Documents (if any) 
6. Recommendations of GP Secy./Ward Commissioner & duly signed by B.D.O/Chairman or chairperson
7. Passport size photograph of the applicant

** Case to be handled within 1 day after the receipt of application. 
** Dealing Assistant should take action before the end of 2nd working day from date of application.

[B][U]II. With Approving Authority [/U][/B]:

[B][U]Description [/U][/B]:
** Approving Authority to review the Application and take appropriate action (Approve / Reject / Verify) 
** within 41 days from the date of the application.

** The Service is to be catered to within 42 working days from the date of receipt of the application.
