1. I am working in an establishment.[/B]
    [ Establishment means one on which the EPF and MP Act, 1952 applies and the employee working should be eligible to be a member.]
  i. I want an advance from my Provident Fund Account.
 ii. I want my LIC Policy to be financed through my PF Account.
iii. I want to settle my Pension Fund as I have crossed 58 years age.
a. I Have Completed 10 Years of Eligible Service.
b. I Have Not Completed 10 Years of Eligible Service as on 58 Years Age.

2. I have left one establishment and have joined another.[/B]

[B]3. I have left the establishment where I was working and have not taken any other job.[/B]
  i. My Age is Below 50 Years.  
  a. I Have Completed 10 Years of Eligible Service.  
b. I have Not Completed 10 Years of Eligible Service as on Date Of Leaving.
 ii. My Age is Over 50 Years but Below 58 Years.
 a. I Have Completed 10 Years of Eligible Service.
 b. I have Not Completed 10 Years of Eligible Service as on Date Of Leaving.
iii. My Age is Above 58 Years.
a. I Have Completed 10 Years of Eligible Service.
b. I have Not Completed 10 Years of Eligible Service as on 58 Years of age.

  i. Member died while in service before 58 years Age.

iii. Member died away from service before 58 years Age.
 iv. Member died away from service after 58 Years Age.
